The shrine sits at the foot of Mount Homan outside Dazaifu. Fans connect it to Demon Slayer through the shared "Kamado" name, the fact that the author is from Fukuoka, and the checked patterns worn by practitioners associated with the mountain. None of this is an official statement — it is fan inference, and the shrine is an active place of worship with its own long history.
Why it is worth it
It is 30 minutes from Fukuoka and pairs with Dazaifu Tenmangu, which you were probably visiting anyway.
When to go
Busy with regular shrine visitors, especially at New Year and in autumn.

This is a functioning shrine, not an anime site. Behave as you would at any shrine, and do not assume staff will engage with the series connection.
